This section includes the various forms made available by the AMF for its diverse clienteles. In certain instances, guides or information documents are also available. Carefully read any related material to ensure you properly fill out all of the necessary forms. 

Downloading forms
Most of the forms on the website are in PDF format. Acrobat Reader® is necessary in order to display and print these documents. If you do not have this software, you can download the latest version free of charge.

Completing forms
Some forms (in Word or PDF format) can be filled out on-line. Note that to complete forms in PDF format, you will need to have a full Acrobat licence. Once you have completed a form, remember to print it before closing the window in which it was displayed.

Printing forms
All forms must be submitted to the AMF in their original format. Make sure your printer is properly configured (8 1/2 x 11 or 8 1/2 x 14) before printing forms.
